To learn more about the indexes please see the links on the panel to the left. To request a search of the indexes download the form below and send it in. WebWhat you need is to contact the Connecticut State Department of Public Health, which keeps death records from 1897 to the present day. The Department also has death indexes from 1949 to the present. If you know where the death occurred, you can request a copy of the death certificate from the town official in the respective location.
